How to Enroll for Direct Deposit

- Complete an EFT/Direct Deposit enrollment authorization form.
- Please write legibly and make sure you Sign and Date the form
- Attach a voided blank check or a letter from your bank to the authorization form.
- If depositing to a savings account ask your bank to give you the Routing/Transit number for your account, this number isn’t always the same as shown on the deposit slip.
- Requests submitted without a voided check or letter from your bank will not be processed
- It is your responsibility to provide complete and accurate account information
- Complete an EFT/Direct Deposit enrollment authorization form.
